Facilitating Home-based Elder Care  

Homebound elderly patients and their caregivers often face significant challenges keeping up with the management of complex, chronic conditions and multiple medications, as well as fragmented care coordination, particularly after hospital visits, according to Tim Weber of Cencora. Long term care at home is the most significant initiative that Good Neighbor Pharmacies has implemented to address the needs of elderly patients and their caregivers. 

Assessing Pharmacist-Patient Communication      

Researchers have assessed the communication concordance between pharmacists and patients during medication counseling. Patients are not always walking away with the right message, pointing to a critical gap between information delivered and retained, according to a study from the University of Texas at Austin College of Pharmacy and Touro College of Pharmacy, and funded by the American Pharmacists Association Foundation.


Modernizing the Pharmacy Workforce

The “2025 Measures Under Consideration (MUC) List” is now available from the Centers for Medicare & Medicare Services on the Measures Management System (MMS) Hub. The publication includes an Excel file detailing measures specifications, along with the “2025 MUC List Overview,” a PDF that summarizes the MUC List process and provides high-level information. The list is relevant for the Merit-based Incentive Payment System and the Medicare Promoting Interoperability Program.
