The third edition of the PQA Social Determinants of Health (SDOH) Resource Guide profiles 40 real-world SDOH services, including eight new initiatives and updates ten from the second edition. As with the previous editions, the guide focuses on SDOH screenings, referrals, and interventions conducted by or involving pharmacists or pharmacies. This edition features an increased focus on food and nutrition security as it relates to medication use quality and highlights tactics employed by successful SDOH programs.
This PQA Quality Forum Webinar featured one of PQA’s Executive Fellows, Kyra Leonard, PharmD, as she provided an overview of the Third Edition of the PQA SDOH Resource Guide. Leonard was joined by Faria Chaudhry, PharmD, from Purdue University. Chaudhry detailed ongoing efforts to improve patients’ cardiovascular care and access to medications and nutritious foods. These initiatives from Purdue University were featured in the third edition of the guide. PQA Director of Education Amanda Ryan hosted the forum and moderated a question-and-answer session with the audience at the end of the webinar.
