Improving Quality Through Health Plan and Pharmacy Value-Based Partnerships

This August 7, 2025, Quality Forum Webinar is an encore presentation from the highest-rated breakout session from the 2025 PQA Annual Meeting.

Value-based care is transforming healthcare by rewarding quality over volume, and pharmacies have a critical role to play. While providers are often the focus, partnerships between health plans and pharmacies can drive meaningful improvements in outcomes for at-risk patients.

Maaria Kheiri, PharmD, Director of Pharmacy Quality Improvement at Centene Pharmacy Services, and Jeff DiGeronimo, BS, Vice President of Program Strategies and Client Success at RxAnte, explored how data-driven strategies, personalized pharmacy care programs, and aligned incentives can support pharmacies in advancing value-based care. They showcased two proven value-based pharmacy models: a community pharmacy incentive program and a comprehensive care management program for complex and vulnerable seniors.

PQA Education Director Tammy Malm hosted the forum and moderated an audience question-and-answer session at the end of the webinar.
